  • Fonterra Welfare Fund Application Form (Existing) 
  • Resolution Life Personal Statement Form

For existing employees wanting to apply for Critical Illness Insurance,, please download and complete the following: 

  • Fonterra Welfare Fund Application Form (Existing) and the AIA Life Personal Statement Short Form.
